The Ocean County Aging Out/ Aging In Committee is proud to announce a resource fair to showcase the programs and services offered for youth between 14 and 22 years of age.

Objectives:

☼ Empower youth and young adults with information to guide their own care
☼ Increase knowledge and access of community resources that address the varied issues experienced by youth in transition
☼ Create a seamless transition, promote resiliency and support wellness and recovery for youth with mental health and behavioral challenges as they become independent adults

Who Should Attend:

Anyone who assists in referral & linkage for youth in transition

In 2007, a joint committee was formed of the CIACC and Mental Health Unit to identify and address the systemic gaps and barriers that exist for youth in transition from involvement in children's services to the adult mental health service system. The mission of the "Aging Out/Aging In Committee" is to foster local system integration, information/ resource sharing, and networking aimed to facilitate successful transition for young adults in Ocean County. The Committee began by providing collaborative case consultation and technical assistance to case workers. In the fall of 2008, Ocean County was granted the opportunity to serve as a pilot county for Transition Planning Conferencing, a statewide initiative of the Division of Child Behavioral Health Services (DCBHS) in partnership with the Division of Mental Health Services (DMHS), which was loosely based on the processes that had began in Ocean County.

The Committee hopes to empower and support youth and young adults' transition to adulthood through this annual resource fair.
